Rather than looking for a rare caldina one, how hard would it be to get a 3sge one to fit?

different port shape, different stud pattern. Ideally you'd want to get the pattern for st205/215, cut the flange off the st16x side feed and then weld on the new flange.
By that time, you may as well have built a new manifold or modified an aftermarket evo one.

This is assuming you are talking about modifying a st16x side feed vs gen3 3sge feed.

If gen3 3sge feed, you can take the bottom portion of the feed and mount a blank manifold to the top, but I haven't seen it done before.

what about the 3sge head + intake manifold onto 3sgte block, then stud pattern and port patern is not an issue (except exhaust side....) may have to change cams over...

Are you saying the ports are different between 3sge and 3sgte on the rev 3 ?

As regards manifold, I know the rev 2 has a double plenum setup with very long runners which come forward over the head. Not sure about the rev 3 though.

AFAIK the GE manifolds will fit the same rev GTE heads

So a rev 3 202 inlet manifold should fit a 205 head. The rev 3 202 is a single plenum side feed - the plenum sits over the cam cover. It also has a secondary plenum which sits roughly where the GTE manifold is. This acts as a resonant chamber and is connected via butterflies to the main inlet runners.
All xx2 manifolds are non starters if you have a topmount cooler in place but since you're investigating side feed I assume this isn't a problem!

I suspect (but don't know) that in a NA setup this resonator plenum might build a small positive pressure at high rpms by channeling fast moving inlet air into the plenum when the inlet valves close. It also gives a long runner at low rpm and short runner at high rpm which is mo betta for pulse tuning

DIY option http://www.rossmachineracing.com/intakepartspage.html...
but likely to be $$$$

Gen3 GE and GTE ports are the same and as noted the side plenum is over the cam cover with the ACIS plenum at the rear.
Not sure how you would control the ACIS butterflys, but you could remove them as per TVIS butterfly removal.

ACIS: http://www.mkiiitech.com/forum/showthread.php?t=312

As to the st202 throttle body, its rather small and the manifold still requires modification if you would want to go to a larger tb opening ie cut off old flange, weld on larger flange for aftermarket unit.

Just pony up and spend some money to get a st215 unit. If you are really stuck, I can call some wreckers here in NZ and sort out pricing, but as I quoted in another post, avg cost is $300 - 400NZ for the plenum and TB and you'd still need to get the 3 pin plug to control the TPS. Add postage and you're done and dusted. I'm not particularly keen to go hunting for parts as it burns a lot of car build time for me and my time is limited, but if there is a call to get some units to the UK, we could look at 2-3 plenums/tb/plugs being sent out to consolidate the cost of shipping. I have shipping account, so not that big of an issue there.

Has anyone ever measured runner spacing of an Evo/4G63 head? Cut flange/runners off ST205 manifold, get one of many aftermarket Evo manifolds, combine... profit?

I have an Evo side feed manifold which I was considering using before I got the Caldina manifold. The runner spacing of the Evo is very close to the Toyota head/port spacing. I didn't use it in the end as I found I could buy a Caldina manifold brand new from MrT for reasonable money.
